In order to save yourself some time please print off the Medical History Form (Adult or Child / Teen) best suited to your visit and complete it prior to arrival.
The Insurance Information Sheet can also be completed ahead of time.
(Don't forget to bring your Insurance card as we will need to make a copy of it.)
The Release of Medical Information form can be completed and signed so that we can request your old records from your previous physician (s).
